I hereby authorize Floral Wonders, LLC to charge my credit card above for agreed-upon purchases. I understand that my information will be saved on file and that future monthly payments will be charged to my credit card until I cancel.
Program participation cancelation can be done by either calling our office at (630) 945-6559 or by emailing hello@floral-wonders.com, and it must be received at least three business days before Monday of the preferred delivery week.

The total monthly charge will consist of the amount indicated in this form plus 8% sales tax.
The flower arrangement delivered as a part of this program will be our arrangement of the month, as pictured on our Flower Club page: https://floral-wonders.com/flowerclub. If you'd rather have a different arrangement delivered for the specific month, please call or email us at least three business days before Monday of the preferred delivery week.
